Key Responsibilities\n

• Conduct comprehensive assessments to identify and understand the social, emotional, and environmental challenges facing clients.

• Develop and implement individualized service plans that set clear goals, outline interventions, and monitor progress effectively.

• Provide therapeutic counseling to clients and their families addressing personal challenges that may include grief, adaptation to chronic illness, or navigating the complexities of end-of-life issues.

• Collaborate actively with clients to create tailored care plans and connect them to essential community resources such as housing support or financial aid.

• Lead crisis intervention efforts to assess clients’ risk levels and develop safety plans when immediate support is necessary.

• Network with community agencies and organizations to ensure coordinated services and robust advocacy for client rights.

Patient Population / Client Focus\n

You will be working primarily with individuals in hospice care, addressing the unique and sensitive needs of clients facing serious illness and their families. Your role will involve providing emotional support, facilitating conversations about end-of-life decisions, and ensuring clients receive the appropriate resources to enhance their quality of life in their remaining days. This client focus requires a deep understanding of grief, loss, and the psychosocial elements surrounding palliative care.

Qualifications\n

• A Master's Degree in Social Work from an accredited program is required.

Compensation and Benefits\n
