Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient service providers to ensure continuity of care.

• Engage with insurance companies to manage pre-certification requirements and facilitate utilization management effectively.

• Develop and participate in comprehensive discharge and aftercare planning meetings, coordinating with healthcare providers to optimize ongoing support.

• Provide therapeutic meal support, emphasizing the modeling of a healthy relationship with food while promoting emotional well-being.

• Offer personalized, empathetic, and affirming care, aligning therapeutic practices with the unique backgrounds and intersecting identities of each client.

• Document and maintain accurate clinical records and progress notes, adhering to the highest ethical standards in social work practice.

• Regularly assess client progress and modify treatment plans as necessary to ensure the best possible outcomes.

Patient Population / Client Focus\n

In this role, you will primarily work with adults, including older adults who may face a multitude of mental health challenges, such as anxiety, depression, and trauma-related disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in the state of Georgia.

Compensation and Benefits\n
