Job Description
Title: Physical Therapist

Schedule: Days, 8-hour shifts

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: Not specified

Requirements:
- 1 year of PT experience (required)
- LTC or rehab experience (required)
- MA State License (required)
- New Grads acceptable with Level II experience, not just Level I observation experience

Roles & Responsibilities:
- Evaluate and rehabilitate patients with physical functioning disorders and/or wound care, developing appropriate plans of care.
- Utilize standardized assessments and evidence-based practices to guide clinical interventions.
- Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, and modalities.
- Assess patient progress and adjust treatments accordingly, documenting in clinical records.
- Provide consultation and counseling to patients, families, caregivers, and service providers regarding physical disorders.
- Maintain effective communication with supervisors and healthcare team members regarding patient treatment plans and progress.
- Document services provided according to regulatory and clinical requirements in the electronic documentation system.

Compliance / Notes:
- Participation in required meetings and training sessions is expected.
- Adherence to relevant practice acts, code of conduct, and ethics during professional practice.
