About the Position

We are seeking a compassionate and dedicated Per Diem Renal Social Worker to join our dialysis team in Chicago, Illinois. The Renal Social Worker will provide psychosocial support to patients receiving dialysis and work closely with the interdisciplinary care team to help patients and their families navigate the challenges associated with End-Stage Renal Disease (ESRD).

This is an excellent opportunity for a Social Worker looking for flexible per diem work in a patient-centered dialysis environment.

Responsibilities

• Conduct psychosocial assessments of dialysis patients and identify social, emotional, financial, and environmental needs.
• Provide individual and family counseling and emotional support.
• Assist patients with adjustment to chronic kidney disease, dialysis, and changes in their lifestyle.
• Participate in interdisciplinary care planning and patient care conferences.
• Develop and implement appropriate psychosocial interventions.
• Assist patients with accessing community resources and social services.
• Provide referrals for transportation, housing, food assistance, financial resources, behavioral health services, and other community programs.
• Assist patients with barriers that may affect dialysis attendance, treatment adherence, and overall well-being.
• Support patients interested in kidney transplantation and home dialysis options.
• Assist with vocational rehabilitation and other resources that promote patient independence and quality of life.
• Provide crisis intervention and support during difficult or stressful situations.
• Assist patients and families with advance care planning and end-of-life concerns when appropriate.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Participate in interdisciplinary meetings and quality improvement activities.
• Maintain patient confidentiality and comply with HIPAA and applicable federal and state regulations.

Qualifications

• Master's Degree in Social Work (MSW) from a CSWE-accredited program.
• Appropriate Illinois social work licensure for the position and assigned duties.
• Previous healthcare, medical social work, dialysis, nephrology, or chronic disease experience preferred.
• Strong communication, counseling, assessment, and case-management skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Compassionate, professional, and patient-centered approach.
• Strong organizational and documentation skills.
