Travel Nurse Case Manager - Full Time

Registered Nurse to provide in-home patient care as part of an interdisciplinary team, including Nurse Practitioners, Behavioral Health Advocates, and Care Navigators. The RN assesses health status, develops and evaluates care plans, and collaborates with other healthcare professionals to manage patient needs. Responsibilities include conducting assessments, counseling, educating patients and families, coordinating referrals and transitions, documenting in electronic medical records, and participating in quality initiatives. The role involves travel across the state to support growth, coverage, and caseload management.
