Key Responsibilities\n

As a Licensed Professional Counselor at Rethink Mental Healthcare, your responsibilities will include:

• Conducting thorough ongoing assessments to evaluate and address the individual needs of patients.

• Maintaining clear communication with supervisors, family members, and other stakeholders regarding patient progress and treatment plans.
\n
• Creating tentative schedules of appointments, home visits, and observations to monitor client progress effectively.
\n
• Coordinating home visits that adhere to in-home observation requirements, alongside chart reviews and staff interviews for comprehensive evaluations.
\n
• Ensuring effective implementation of individualized program plans tailored to client needs, and making necessary adjustments as required.
\n
• Actively participating in interdisciplinary team meetings and contributing to collaborative discussions on patient care.
\n
• Completing detailed monthly and quarterly progress notes to document patient advancements and treatment efficacy.
\n
• Organizing training sessions to empower staff and team members in the implementation of treatment protocols.
\n
• Providing follow-up support through interviews, home visits, and ongoing observations to meet clients’ evolving needs.

• Maintaining organized records that ensure information accessibility for authorized parties involved in patient care.

• Serving as a liaison between our organization and the broader mental health community to support development goals.

• Assisting in refining training materials and procedures to enhance program delivery for individuals.

Patient Population / Client Focus\n

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.

• A minimum of two years of counseling experience, preferably in a clinical setting.

Preferred Experience\n
