This position is tailored for remote work, offering flexibility while ensuring that our social workers are equipped with the tools to effectively serve our clients from the comfort of their homes. While the role primarily involves telehealth services, you may occasionally need to visit clients in their homes or facilitate meetings in the community, thus occasionally requiring evening or weekend hours as dictated by client needs. Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, utilizing structured tools and individualized interviews to capture essential emotional and social factors.

• Develop, implement, and evaluate personalized care plans in collaboration with clients and their families, tailoring approaches to ensure they meet specific goals.

• Provide ongoing counseling and emotional support to seniors and their families, assisting them in navigating life changes and coping with challenges such as loss, chronic illness, and transitioning to assisted living arrangements.

• Act as an advocate for clients, ensuring they have access to community resources such as food programs, healthcare services, and support groups, while empowering them to voice their needs.

• Collaborate with a multidisciplinary team of healthcare and social services professionals to plan and coordinate comprehensive care strategies for clients.

• Regularly visit clients' homes when necessary to assess their living conditions, safety, and overall well-being, making recommendations for improvements or additional services as needed.

• Facilitate communication among family members to resolve conflicts, provide psychoeducation on the roles of each family member, and enhance collaborative care.

• Educate clients and families about local resources, social programs, and support networks to promote social engagement and community involvement.

• Maintain meticulous records of client interactions, progress notes, and adjustments to care plans in electronic health record (EHR) systems, ensuring compliance with ethical standards and best practices.

• Stay updated on social work practices, compliance issues, and policies that affect the elderly population.

Patient Population / Client Focus\n

The primary focus of this role is on senior clients and their families, addressing a range of psychological, emotional, and social challenges specific to aging. You will work with clients experiencing various life transitions, such as retirement, health changes, and the loss of loved ones, and you will be instrumental in helping them navigate these transitions with dignity and respect.

Qualifications\n

• A Master’s degree in Social Work from an accredited university.

• Current and valid licensure as a Master Social Worker (LMSW) in the state of Florida.

Compensation and Benefits\n
