Work Environment\n

This role offers a flexible, remote work setting, primarily based in a school environment. Our model enables social workers to engage with clients and their families through virtual platforms, allowing for both scheduled visits and on-demand support. The typical working hours are from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as required to meet client needs. You will need to demonstrate flexibility in your schedule to accommodate home visits and ensure comprehensive care delivery.

Key Responsibilities\n

Your day-to-day activities will include, but are not limited to, the following:

• Conduct exhaustive assessments of psychosocial needs, factoring in emotional, mental, and social influences affecting seniors.

• Collaborate with clients and their families to devise individualized care plans that align with their personal goals and preferences.

• Provide counseling and emotional support during critical life transitions, enabling clients to manage challenges with dignity and confidence.

• Act as an advocate for clients, ensuring they gain access to available community resources and appropriate services.

• Work in harmony with interdisciplinary teams comprising nurses, caregivers, and healthcare professionals to ensure cohesive and enhanced care delivery.

• Perform regular client visits to evaluate living conditions, monitor well-being, and proactively address safety issues.

• Facilitate effective communication among family members involved in care coordination, resolving conflicts when necessary.

• Maintain meticulous documentation of assessments, care plans, progress notes, and any relevant interactions within electronic health record (EHR) systems.

Qualifications\n

Compensation and Benefits\n
