Bright Path Social Work is seeking an empathetic and experienced Licensed Master Social Worker (LMSW) to join our dedicated team in Aurora, Illinois. Work Environment\n

This position is primarily remote, offering flexibility while working from home, which is particularly beneficial to accommodate the various schedules of our clients. However, occasional visits may be required to a Detox Center where services are delivered. You will collaborate daily with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to ensure a comprehensive approach to client care. Our hours of operation are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend commitments based on client needs.

Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, evaluating emotional, mental, and social factors to develop comprehensive care strategies.

• Collaborate with clients and their families to create personalized care plans that focus on individual goals and preferences, ensuring dignity and respect are maintained throughout the care process.

• Provide counseling and emotional support to seniors and their families, facilitating discussions that address life transitions and support their coping strategies effectively.
\n
• Advocate for clients' rights and access to vital community resources, ensuring they receive appropriate support and services needed for their well-being.

• Work closely with an interdisciplinary team to enhance client care, ensuring comprehensive service delivery aligned with clinical and social needs.
\n
• Perform regular home visits to monitor and address clients' living conditions, incorporating safety assessments into your evaluations.
\n
• Facilitate family communication and conflict resolution regarding care, ensuring all parties are informed and supported in their roles.

• Educate clients and their families about community resources, support groups, and programs that promote social engagement and enhance quality of life.
\n
• Maintain accurate documentation of client assessments, care plans, and progress notes in compliance with organizational standards and regulations.

• Stay current on social work practices, policies, and regulations, ensuring professional compliance and ethical standards are upheld.

In this role, you will primarily work with seniors, many of whom may be facing challenges associated with aging such as chronic illness, mental health issues, or the emotional repercussions of significant life changes. Your expertise will be critical in promoting their overall well-being, respecting their autonomy, and fostering their independence while also providing necessary support.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A current and valid state licensure as a Licensed Master Social Worker (LMSW) is essential.

• At least two years of social work experience, particularly in a community setting or geriatric population, is positioned as a strong aspect of your application.

• Proficient knowledge of social work principles, aging-related issues, and resources available within the community.

• Demonstrable assessment, counseling, and problem-solving skills, combined with a deep sense of empathy and compassion.
