Key Responsibilities\n

• Participate actively in interdisciplinary team meetings, contributing insights and collaborating on patient care strategies.

• Document monthly and quarterly progress notes accurately, reflecting each patient's journey and interventions in accordance with regulatory standards.

• Organize training sessions for team members to ensure consistent methodologies in program plan implementation and documentation practices.

• Provide ongoing support through follow-up interviews and home visits, ensuring continued alignment with individual needs.

Patient Population / Client Focus\n

Qualifications\n
