Key Responsibilities\n

• Conduct thorough assessments to determine the needs of clients and formulate individualized treatment plans that address their unique situations.

• Maintain regular communication with supervisors, direct care staff, and other key stakeholders regarding client progress and treatment adjustments.

• Provide a consistent appointment schedule to supervisors, ensuring that home visits, observations, and follow-ups are systematically organized.

• Coordinate and conduct home visits, gathering data through observations and interviews to keep a comprehensive view of client needs.
\n
• Implement and monitor program plans to ensure effectiveness and adherence to best practices.
\n
• Engage actively in interdisciplinary team meetings to discuss treatment strategies and collaborate with colleagues for better client outcomes.

• Document monthly and quarterly progress notes to track client growth and inform necessary adjustments in care.
\n
• Organize and facilitate training sessions for team members to ensure competence in program implementation and documentation.

• Conduct follow-up interviews and home visits to offer ongoing support and assess service effectiveness.

• Serve as a liaison with the community and mental health organizations to support departmental goals and enhance service accessibility.

You will work with a diverse patient population, including children, adolescents, and adults dealing with various mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related therapeutic field is required.
