Key Responsibilities\n

• Conduct thorough and ongoing assessments to identify the unique needs of each client, ensuring a tailored approach to their treatment plan.

• Engage in regular communication with supervisors, clinical staff, and family members to formulate and review individual progress reports.

• Prepare and present a structured schedule of client appointments and home visits to keep supervisors informed about your caseload management.

• Coordinate home visits as necessary for in-home observations and participant evaluations, actively documenting findings and recommendations.

• Ensure treatment plans are implemented effectively, monitoring client progress and adjusting interventions as needed to promote optimal outcomes.

• Participate in Interdisciplinary Team Meetings, contributing insights on clinical practices and collaborative approaches to enhance client care.

• Complete monthly and quarterly progress notes, providing comprehensive updates on client status and interventions implemented.
\n
• Organize and conduct training sessions to support staff development and program fidelity related to treatment methodologies and documentation standards.

• Provide follow-up interviews and home visits to assess ongoing client needs, creating a supportive environment for growth and progress.

• Maintain accurate and confidential client records, ensuring compliance with legal and ethical standards of practice.

Qualifications\n
