Key Responsibilities\n

• Conduct ongoing assessments of patients to understand their individual needs and develop tailored treatment plans.

• Facilitate effective communication among supervisors, healthcare staff, family members, and other relevant parties regarding each patient’s progress and care.
\n
• Regularly schedule appointments, home visits, and observations to ensure comprehensive support for clients and document these interactions accurately.
\n
• Coordinate home visits, ensuring they meet in-home observational requirements, and gather relevant data through chart reviews and staff interviews.
\n
• Monitor and ensure the effective implementation of treatment programs, making any necessary adjustments to meet patient needs.
\n
• Participate in interdisciplinary team meetings to discuss patient progress, treatment strategies, and collaborative efforts.
\n
• Prepare and complete monthly and quarterly progress notes, tracking patient advancements and outcomes.
\n
• Organize and conduct training sessions to ensure that all staff are equipped to implement program plans consistently and accurately.
\n
• Provide continuous support through follow-up interviews and home visits, ensuring that patient needs are met and revisited regularly.
\n
• Maintain thorough documentation to guarantee that patient information is easily accessible to authorized individuals.
\n
• Act as a liaison with the public and the mental health community to assist with program development and community outreach initiatives.

• Contribute to the creation of training materials and procedures that enhance the delivery of our mental health programs.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Tennessee.

• Minimum of two years of clinical counseling experience, preferably within a healthcare or clinical setting.

• Strong understanding of legal and ethical standards governing mental health practice.

• Exceptional interpersonal and communication skills, with the ability to build rapport across diverse populations.

• Proven ability to collaborate effectively within a healthcare team setting.

• Familiarity with electronic medical records (EMR) software and its application in documenting patient care.

Preferred Experience\n

• Specialized training or experience in family therapy, couples therapy, trauma-informed care, or treatment for substance misuse is highly preferred.
