Key Responsibilities\n

• Engage with clients through various means—both virtually and telephonically—to provide therapeutic support for those experiencing major mental illnesses and severe emotional disorders.

• Conduct comprehensive needs assessments and develop individualized treatment plans addressing issues relevant to each client’s unique circumstances.

• Facilitate individual, group, and family therapy sessions aimed at addressing specific therapeutic goals and enhancing client coping strategies.

• Document and monitor client progress to ensure alignment with treatment objectives, maintaining accuracy in managed records and reports.

• Work collaboratively within a multidisciplinary team to coordinate care and ensure comprehensive support across all client facets.

• Participate in on-call rotations, providing crisis intervention services and guidance as needed to ensure client safety.

• Be active in agency committees focused on enhancing service delivery and advancing quality improvement initiatives.

Patient Population / Client Focus\n

Your primary focus will be on vulnerable populations within the hospice center that may present complex emotional and psychological needs. This includes working closely with clients facing terminal illnesses, grief, and various stages of emotional turmoil. You will play a crucial role in advocating for their needs and facilitating access to the critical resources they require for support.

Qualifications\n

• Must possess a valid and unrestricted LCSW license in the state of New York.

• A Bachelor’s degree in social work or a closely related field; additional degrees or certifications are preferred.
