In this role, you will:

• Assess patients in both inpatient and outpatient settings and develop tailored treatment plans that reflect individual needs.
• Implement physical therapy interventions that enhance mobility, strength, balance, and functional abilities.
• Work in collaboration with a multi-disciplinary team, including nurses, physicians, case managers, and fellow rehabilitation specialists, to maximize patient outcomes.
• Provide education to patients and their families about therapeutic goals, discharge planning, and at-home exercises to encourage continued recovery.
• Document evaluations, treatment sessions, and progress meticulously, adhering to facility and regulatory protocols.
• Participate in care team discussions and interdisciplinary meetings to ensure cohesive patient management.

To thrive in this role, you should possess:

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
• Valid state licensure as a Physical Therapist.
• CPR certification preferred, along with any specialized certifications (e.g., neurological, geriatric) considered advantageous.
• A track record of clinical experience, especially in managing complex medical cases.
• Strong communication and teamwork abilities paired with a comfort level for using digital charting and clinical documentation systems.
