If you're a CNA, CHHA, or HCA with strong people skills and a desire to make a difference beyond direct caregiving, this role offers an exciting opportunity to impact both clients and caregivers while growing your career in home care leadership.Why you will love this role:Build meaningful relationships with clients and familiesMentor and support caregivers in the fieldConduct client assessments and quality assurance visitsCoordinate care with hospitals, rehab facilities, and skilled nursing facilitiesMake a direct impact on client satisfaction and caregiver successGrow your leadership skills in a rapidly expanding organizationWhat you'll be doing:Client Assessments & Care PlanningMeet with prospective clients and families to assess care needs.Develop personalized care plans.Coordinate service starts and work with scheduling to ensure proper staffing.Provide hands-on caregiving support when operational needs require.Hospital & Facility Care CoordinationCoordinate transitions from hospitals, rehabilitation centers, and SNFs.Collaborate with social workers, discharge planners, case managers, and family members.Help reduce hospital readmissions through proactive follow-up.Quality Assurance & Client SatisfactionConduct satisfaction calls and in-home quality visits.Perform reassessments and update care plans.Ensure Assignment of Benefits forms are completed and maintained.Caregiver Coaching & SupportConduct field visits and caregiver observations.Assist with onboarding and training.Provide coaching and additional education as needed.Support caregiver engagement and retention.Client & Family Relationship ManagementServe as a trusted resource for clients and families.Respond to concerns with empathy and professionalism.Encourage positive reviews and referrals.Required qualifications:Active CNA, CHHA, or HCA credentialHigh School Diploma or GEDValid driver license, reliable vehicle, and proof of insuranceMinimum one year of caregiving experienceComfortable traveling throughout the service areaPreferred qualifications:Minimum one year of caregiving experience in home care, home health, assisted living, memory care, skilled nursing, or related healthcare setting.Experience performing client assessments and creating care plansExperience working with hospitals, discharge planners, case managers, or social workersExperience training, mentoring, or leading caregiversThe ideal candidate:Compassionate, organized, service-oriented, a natural relationship builder, comfortable working independently, and committed to exceptional care and customer service.Benefits of joining Safe Harbor Home Care:Competitive payCareer advancement opportunitiesSupportive leadership teamCollaborative cultureMeaningful work that impacts lives every dayApply todayIf you're a dedicated CNA, CHHA, or HCA looking for a career that combines caregiving, client advocacy, quality assurance, and team support, we'd love to meet you.Work schedule:8 hour shiftMonday to FridayBenefits:Paid time off401(k)Paid trainingMileage reimbursement
