This position serves as the primary admissions nurse Friday through Monday.Key ResponsibilitiesComplete home health admissions and Start of Care (SOC) assessments in accordance with physician orders and agency policiesObtain and document comprehensive medical historiesPerform physical assessments, including vital signs, mobility evaluations, and safety assessmentsComplete OASIS documentation accurately and within required timeframesDevelop individualized plans of care based on patient needs and physician ordersEducate patients and caregivers regarding diagnoses, medications, treatment plans, and home health servicesCoordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team membersEnsure compliance with all federal, state, and agency regulations, including HIPAA requirementsIdentify and communicate changes in patient condition to the appropriate healthcare providersFacilitate referrals for additional home health services as neededMaintain timely and accurate clinical documentationQualificationsCurrent and active Registered Nurse (RN) license in the State of OhioMinimum of one year of nursing experience required; home health experience preferredOASIS documentation experience strongly preferredExperience with WellSky (Kinnser) preferredStrong clinical assessment and critical thinking skillsExcellent communication and patient education abilitiesCurrent CPR certificationValid driver's license, reliable transportation, and automobile insuranceCurrent TB test and physical examination in accordance with agency requirementsAbility to attend orientation and training at the Aspire Home Healthcare of Ohio officeJoin Aspire Home Healthcare of Ohio and make a meaningful impact by helping patients receive quality care where they are most comfortable—at home. #J-18808-Ljbffr Aspire Home Healthcare of Ohio
