The ideal candidate will hold either an LMFT, LPC, or LCSW license and have a minimum of two years of relevant experience.

Our team operates under a structured schedule, typically Monday through Friday from 8:00 AM to 5:00 PM, with occasional evening shifts required based on patient needs. You will participate in interactive team meetings and case discussions, allowing for a collaborative approach to patient care.

- Conduct thorough mental health assessments, integrating evaluations of patients’ physical health conditions to gain insight into their mental and emotional well-being. \n
- Develop and implement personalized, evidence-based treatment plans that address the unique psychiatric and medical needs of each patient simultaneously. \n
- Facilitate both individual and group therapy sessions, employing various evidence-based therapeutic modalities tailored to the circumstances affecting your patients. \n
- Collaborate with medical professionals to ensure comprehensive care coordination, enhancing treatment outcomes for all patients. \n
- Provide crisis intervention and immediate support during mental health emergencies to stabilize patients and ensure their safety. \n
- Educate patients about the relationship between their mental and physical health, empowering them with strategies for managing their conditions. \n
- Accurately maintain documentation and case notes in compliance with all legal and ethical standards, ensuring patient confidentiality is upheld at all times. \n
- Actively participate in case conferences and interdisciplinary care planning sessions to foster a collaborative team approach. \n
- Continuously update your knowledge of best practices in mental health and integrated care, utilizing ongoing research to inform your practice. \n
You will primarily work with patients experiencing a combination of mental health and medical issues, understanding that many individuals have co-occurring conditions. This includes those dealing with chronic pain, substance use disorders, and other complex medical conditions that impact their mental health. Your role is crucial in bridging the gap between mental and physical health, thereby enhancing patients' overall quality of life.

- A Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution. \n
- A current and valid state license as an LMFT, LPC, or LCSW. \n

- Demonstrated experience in delivering integrated care and collaborating with a multi-disciplinary health team. \n
- Strong interpersonal and communication skills to effectively engage with patients and colleagues. \n

- Familiarity with Electronic Health Record (EHR) systems and proficiency in using computer applications in clinical settings. \n

The compensation for this role ranges from $80,000 to $104,000 annually, commensurate with experience and qualifications.
