Experience conducting comprehensive assessments of patients. Description: This position requires current RN licensure in Client. Candidates should have two years of experience in a home health setting within the past fifteen years. Recent experience conducting comprehensive assessments of patients is essential. Nurses must have experience working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Familiarity with conducting assessments similar to UAS, such as OASIS, is required. Experience providing primary care to homebound individuals is also necessary. Care management for individuals receiving home and community-based services, including Hospice and Assisted Living, is preferred. Candidates should have experience with a minimum of two patients per week. Additional experience in conducting UAS-NY assessments is preferred, especially in the last 365 days. Experience caring for a geriatric population and managing chronic diseases in the home setting is beneficial. Home care experience as a paraprofessional or as a Licensed Practical Nurse caring for at least two patients per week is also relevant. The job involves performing initial in-home pre-evaluations of patients, including assessment of health status and care needs. Candidates will review consumer medical documentation and health referral forms relevant to cases. Data must be entered into an electronic evaluation form and transmitted as required. It is critical to document any concerns or conflicting information that arises during evaluations. Continuity of care must be emphasized to reduce fragmentation and gaps in treatment plans. Discussions with consumers on health care options, needed supports, and service vendor options are part of the role. Initiating communication between consumers and care teams for enrollment into managed care health plans is essential. Candidates must enhance communication and collaborative relationships with interdisciplinary care team members. Collecting quality review data and documentations to support outcome measurements is required. Identifying opportunities for health promotion and illness prevention is an important aspect of the role. Maintaining knowledge of community resources and service network options for the target population is necessary. Performing other duties as assigned by management may also be required. All candidates must be prepared for hybrid tele and in-field assessments. Sensitivity to the concerns of patients and the ability to work in a fast-paced environment are crucial. Candidates should demonstrate strong communication skills and the ability to work independently.
