We are currently seeking a Care Coordinator Supervisor to join our OhioRISE program. As a Care Coordinator Supervisor, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You'll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community.Key ResponsibilitiesThe Care Coordinator Supervisor will guide, support and evaluate the Care Coordinator's individual efforts by focusing on ongoing clinical and compliance issues, staff development, concurrent clinical review and assistance to Care Coordinator with both daily caseload and crisis management.Maintains knowledge of Medicaid, of the behavioral health system and of community resources.Develops, monitors, and updates individual's person-centered care plans to ensure the individual's needs, preferences, health, and welfare.Assists individuals with connections to needed resources, including services, medical, social, educational, and other resources, regardless of funding sourceIdentifies, reports, and addresses all incidents as defined in the program specific guidelines, following the process outlined by program specific guidelines.Documents all care coordination activities in a record to meet all requirements, including accuracy, timeliness, and content requirements using the process prescribed by the program specific guidelinesMinimum QualificationsPossesses an active license as a Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse; AND/OR Bachelor's Degree in Nursing, Social Services or related field with a minimum of 2 years of full-time direct service experience.
