Work Environment\n

This role will be remote, allowing you to provide services from the comfort of your home. The position requires a full-time commitment, typically from Monday to Friday, during standard business hours (8:00 AM to 4:30 PM). However, flexibility is key, as you may need to accommodate occasional evening or weekend hours based on client needs. As a member of our team, you will use innovative digital tools to remain connected with clients and other interdisciplinary team members, ensuring continuity of care.

Key Responsibilities\n

The Clinical Social Worker will take on a variety of essential tasks, including:

• Conducting comprehensive assessments to evaluate seniors’ psychosocial needs, addressing mental health, emotional stability, and social supports.

• Developing tailored care plans in collaboration with clients and families that reflect their specific goals and preferences.
\n
• Providing one-on-one counseling and emotional support, assisting clients through life transitions, grief, and other challenges.

• Advocating for clients' rights and facilitating access to essential community resources, services, and support systems.

• Working alongside nurses and healthcare professionals to ensure a cohesive, interdisciplinary approach to care.

• Conducting regular quality assurance checks through client visits to monitor well-being and adapt care plans as needed.

• Educating clients and families about available resources to enhance social engagement and uplift their overall quality of life.

• Maintaining meticulous documentation of assessments, care plans, progress notes, and communications in accordance with ethical standards.

Patient Population / Client Focus\n

The primary focus of this role will be on senior citizens, who face a myriad of challenges including health issues, isolation, and the transition to assisted living environments. Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from a Council on Social Work Education (CSWE) accredited program.

• At least two years of professional experience in a social work setting, with preference given to those with experience working with senior populations.

Compensation and Benefits\n
