Job Description
Job Title: Clinical Care Registered Nurse

Experience: Minimum of one year working with the frail, elderly, or long-term care population.
License: RN Nursing Diploma or Degree from accredited School of Nursing or University.
Certifications: Current CPR certification by AHA or willingness to obtain.


Must-Have:
- Bachelor's degree preferred in Science of Nursing.
- Minimum of one year home care experience preferred.
- Minimum of one-year wound care experience preferred.
- Reliable transportation and current, valid driver's license with car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated or willing to be fully vaccinated against specified communicable diseases.


Description:
The Clinical Care Registered Nurse role provides direct nursing care to participants.
This role works independently while applying advanced nursing knowledge and skills.
The nurse is able to manage complex clinical situations.
Collaboration occurs with both licensed and non-licensed personnel.
Key responsibilities include triaging emergent situations while remaining independent in nursing practice.
Monitoring acute and chronic medical conditions in coordination with Primary Care Providers is essential.
Utilizing care plans independently to provide clinic and home-based direct care is a fundamental task.
The role involves administering treatments, procedures, and medications as prescribed.
Home visits are conducted regularly to provide care and treatment as ordered by the Primary Care Provider.
This may include wound care, lab draws, nurse assessments, and other nursing tasks.
The work environment entails a visible presence, subject to frequent interruptions.
Communication with staff, participants, caregivers, and other organizations is necessary.
The nurse may work beyond normal hours and on weekends and holidays, as required.
