Hospital Outreach Manager (account manager) - Missouri or Oklahoma

Position Overview Hospital Outreach Manager works to keep DaVita the provider of choice to hospitals. Engages renal discharge planners and case managers to ensure a seamless transition for newly diagnosed and hospitalized dialysis patients. Location & Travel St. Louis, Missouri or surrounding areas; Tulsa, Oklahoma City, OK or surrounding areas. Travel required: 40% within home state, 20% non‑local. Work from home when not traveling. Candidate must reside in one of the listed locations. Essential Duties Analyze data insights and highlight importance to drive growth. Demonstrate executive presence delivering insights to upper leadership. Leverage strategic mindset for root cause analysis and identify opportunities. Consistently outreach to discharge planners to provide education and build awareness of offerings. Prioritize key insights and opportunities to align with business goals and stakeholder needs. Identify opportunities to grow as provider of choice through relationship building and addressing feedback. Partner with internal field teammates to optimize admission process and implement best practices. Combine analytical rigor with strategic thinking to translate feedback into growth‑oriented strategies. Assist with necessary communication between hospital, admission and clinic teammates to work through patient placements with extenuating circumstances. Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers. Develop comprehensive knowledge of local markets. Salary The Salary Range for the role is $78,000.00 - $119,000.00 per year. Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
