Key Responsibilities

• Provide physical therapy treatments to residents under the supervision of a licensed Physical Therapist
• Implement individualized treatment plans in a skilled nursing facility setting
• Monitor and document patient progress and report findings to the supervising Physical Therapist
• Educate patients and caregivers on therapeutic exercises and home programs
• Collaborate with interdisciplinary care teams to support resident rehabilitation goals

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Arizona
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Prior experience working in a skilled nursing or long-term care setting

Preferred Experience / Skills

• Experience with geriatric patient populations
• Familiarity with electronic medical records (EMR) systems
• Strong communication and teamwork skills in a multidisciplinary environment
