Job Summary

The Housing Advocate/Community Health Worker will provide eviction prevention support, housing search support, referral services and follow up monitoring to patients experiencing housing insecurity. This role involves a variety of administrative and field activities to coordinate applications to housing resources, referrals to agencies and develop housing plans.

Responsibilities

Manage, track, and prioritize incoming housing-related referrals to ensure timely patient outreach and support.


Collect and secure required enrollment program documentation.

Provide crisis intervention, advocacy, care coordination, follow-up support, information, and referrals to patients in person, by telephone, and through other communication methods as appropriate.


Accompaniment to housing court and mediations.


Conduct comprehensive assessments of housing barriers, and available resources to determine appropriate housing and supportive service interventions.


Develop individualized housing search plans, assist with housing applications, and gather and prepare required documentation for landlords, housing authorities, and community agencies.


Research and maintain knowledge of community resources and programs, including housing assistance, legal services, and funding sources.

Facilitate connections to community-based providers and services, including accompaniment to appointments and resource sites when necessary.


Deliver culturally responsive, trauma-informed, and patient-centered services to individuals and families from diverse backgrounds and communities.


Document patient interactions, assessments, interventions, and outcomes in accordance with organizational, programmatic, and regulatory requirements.

Attend team meetings and collaborate across programs and workstreams to support coordinated service delivery, cross-coverage, and operational effectiveness.


Perform other duties and responsibilities as assigned.

Qualifications

Qualifications

• 3-5 years experience working in human services, particularly within housing stabilization.
• Associate or bachelor's degree preferred.
• Demonstrated experience successfully working with diverse and historically underserved populations.

Skills

• Resident of the local community with knowledge of community-based resources and services.
• Experience as a community health worker, advocate, outreach worker, housing advocate, case manager, property manager, or in a related role preferred.
• Knowledge of housing systems, including subsidized and private housing resources.
• Experience working with individuals and families experiencing homelessness or housing instability.
• Ability to work independently, prioritize responsibilities, meet deadlines, and manage a flexible workload with minimal supervision.
• Sustain partnerships with housing agencies, community-based organizations, and other stakeholders to support resource identification and referral processes.
• Comfortable conducting community outreach, field visits, and accompanying patients to appointments or advocacy-related activities (e.g., housing court).
• Ability to exercise sound judgment, manage follow-up activities, and navigate complex human services systems.
• Experience using motivational interviewing and other person-centered engagement approaches preferred.
• Strong verbal and written communication, interpersonal, and relationship-building skills.
• Commitment to ongoing learning and professional development, with an interest in best practices and emerging trends in housing and community health.
• Fluency in Spanish preferred.
