Nurse Practitioner (NP)

Nurse Practitioner (NP) STGi is currently seeking a Nurse Practitioner to provide services at our Community Based Outpatient Clinic. Job Summary The Nurse Practitioner provides professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) Primary Care Provider (PCP). As an Advanced Practice Registered Nurse, the NP offers a wide range of health services, including urgent and emergent evaluation of episodic and chronic problems, comprehensive primary health care, prevention, prescribing medications, ordering and interpreting laboratory and diagnostic studies, and patient education using community, social, and health resources. Essential Functions Provide primary and preventive medical care under CPT codes within scope and training. Provide patient care per nationally recognized APRN standards. Act within scope of the New York Nurse Practice Act, Joint Commission, VA guidelines and Officer of the Inspector General standards. Work closely with the Patient Aligned Care Team (PACT) and support staff, including daily huddles. Emphasize wellness, prevention, and early detection. Supervise support staff in delivery of care. Maintain universal precautions and infection control practices. Ensure smooth, timely flow of patients per VA access standards and triage protocol. Perform CPR and assist during respiratory and cardiac arrest procedures. Investigate health problems by gathering and assessing historical information. Perform a physical examination appropriate to the patient's history. Diagnose health problems based on history, exam findings, and diagnostic test results. Develop a tailored treatment plan including health education, pharmacologic and non‑pharmacologic treatment. Prescribe medications appropriately. Evaluate treatment results and modify plan to achieve goals. Make referrals for specialty evaluation or treatment. Direct and coordinate health care services for walk‑in patients. Provide a model for clinical practice within the clinic setting. Set up and operate therapeutic and emergency equipment (monitor, ECG, oxygen, AED, portable suction). Obtain patient history and record findings per VA standards. Perform initial and periodic physical examinations and record findings. Order diagnostic tests based on complaint, physical and emotional findings. Demonstrate knowledge of pharmacology. Prescribe and administer medication within privileges and regulations. Recommend and follow up on laboratory and diagnostic procedures as needed. Document all patient information in VISTA to demonstrate quality care and promote continuity. Keep the VISTA clinic reminder tracking system current for each enrolled patient. Close patient charts within 24‑72 hours of encounter and place return‑to‑clinic orders promptly. Assist in scheduling initial and follow‑up appointments per VA access standards and patient needs. Keep patients informed of their care plan, including lab and diagnostic results. Provide health educational materials and resources to patients and families as appropriate. Assist the Registered Nurse to ensure documentation is complete and accurate. Comply with all federal, state, local, Joint Commission, OSHA, Veterans Affairs, and subcontractor safety regulations. Participate in the Performance Improvement Program between VAMC and STGi including peer reviews. Ensure all required reports and clinical information are completed accurately, timely, and completely. Participate in staff development programs. Maintain confidentiality of all information and support patient privacy, rights, and safety. Aid in management/coordination of the CBOC's day‑to‑day activity. Perform other clinical duties as assigned within privileges. Required Skills Three years of clinical nursing experience (minimum one year as APRN; three years preferred). Experience in outpatient care within Family Medicine or Internal Medicine preferred. Current and unrestricted DEA license; must obtain and maintain a Scope of Practice/Collaborative Agreement with a state‑licensed physician. Current and unrestricted New York RN license, complying with all state requirements. BLS/CPR certification from American Heart Association or Military Training Network. Prescriptive authority in accordance with state rules and regulations. Ability to set realistic, measurable education goals for patients. Ability to develop and provide individualized and group counseling focused on disease prevention and health maintenance. Experience with safe opioid medication practices for chronic pain. Working knowledge of Microsoft Office; excellent computer maneuverability. Exceptional diplomatic communication skills and experience handling difficult customer situations. Detail oriented with ability to multitask. Required Education Master’s degree in nursing from an NLN/AC or CCNE accredited program. Current certification as a Nurse Practitioner from the American Nurses Association (ANA). ANA certification in Adult Health or Family Practice. Working Conditions / Physical Requirements Moderate physical activity; handle objects up to 25 pounds; stand/walk >4 hours/day. Work in a typical interior/office environment. Respond to medical emergencies requiring rapid movement or assistance.
