Social Services Advocate
Investigate and resolve complaints made by or on behalf of residents living in long-term care facilities. Provide advocacy and mediation to ensure resident rights and quality of life are maintained.
Key Duties
- Conduct regular, unannounced visits to residents in long-term care facilities
- Identify, investigate, and resolve resident complaints
- Provide education, advocacy, and mediation on resident rights
- Maintain professional relationships with nursing and board & care facility staff
- Document case management activities accurately and in a timely manner
Requirements
- Bachelor's degree in social work, social services, gerontology, or related field — OR three or more years of related experience in LTC advocacy, social services, case management, or conflict resolution
- Certified as a State Long-Term Care Ombudsman or the ability to successfully complete the 36-hour State Certified Long Term Care Ombudsman training.
- Strong communication, problem-solving, and case documentation skills
- Valid CA driver's license, auto insurance, and personal vehicle; ability to travel 60% within Riverside County
- Bilingual Spanish a plus
- Must pass LiveScan background check and provide proof of negative TB status
Additional Information
Candidates must not have worked in a skilled nursing facility, assisted living facility, or board and care facility within the past 12 months. INDH
