Clinical Social Worker - Discharge & Care Coordination

Ochsner Health is seeking a Social Worker to conduct bio-psychosocial assessments, coordinate discharge plans, and connect patients with community resources. You will collaborate with a multidisciplinary team and may practice clinically under supervision of a licensed social worker. Requires an MSW and CSW/RSW certification, with LMSW preferred; solid data-entry and documentation skills are essential to support patient care and compliance. #J-18808-Ljbffr
