Piedmont is seeking a Care Manager in Atlanta, Georgia responsible for care coordination, proactive discharge planning, and ensuring timely discharges for patients in their case load. The ideal candidate must be a graduate of a nursing program, hold a current unrestricted RN license in Georgia, have two years of experience in an acute setting, and possess certification in Case Management.
