Care Coordinator & Social Worker: Discharge Planning
Northside Hospital
Northside Hospital is seeking a Care Coordinator to collaborate with patients, families, and clinical teams to evaluate the appropriate care setting and hospitalization needs. The role includes contributing to discharge planning, psychosocial assessment, and utilization review within a progressive acute care environment. Demonstrated case management experience is highly valued.
