Project Coordinator - Health Care LA, IPA (HCLA)

Summary

Health Care LA, a large non-profit network of community health centers serving patients in Los Angeles County, seeks a Project Coordinator to support, under the direction of the Chief Operating Officer (COO), the coordination of initiatives that implement technology solutions, improve quality enhancement efforts, and boost operational efficiency for Health Care LA, IPA, and its 30-member health centers, in collaboration with HCLA’s Managed Services Organization (MSO), MedPOINT Management. The Project Coordinator also provides day-to-day coordination and oversight of HCLA’s Navigator Program, ensuring program activities, schedules, and deliverables stay on track and reporting progress to the COO.

Duties and Responsibilities:

• Under the direction of the COO, coordinate cross-departmental projects and technology-driven initiatives, supporting alignment with HCLA’s strategic objectives.
• Create and maintain a project tracker and calendar; support the development of detailed project schedules using project management tools such as Agile or PDSAs, escalating scheduling conflicts or resource needs to the COO.
• Track the creation and implementation of deliverables, policies, training materials, workflows, and reporting dashboards, flagging timeline or quality concerns to the COO for direction.
• Coordinate day-to-day operations of HCLA’s Patient Navigator Program. HCLA’s Patient Navigator Program is in place to provide outreach support to HCLA health centers for making outbound calls to members to schedule appointments to close gaps in care. The Project Coordinator would provide support to run the Navigator program, including creating and tracking navigator schedules, activity tracking, and ensuring that program deliverables align with organizational initiatives. The Project Coordinator also serves as the first point of contact for routine Navigator Program matters and escalate program issues to the COO.
• Compile Navigator Program data and prepare regular program status reports for review by the COO.
• Support communication with the health centers on project-related collaborations.
• Assist in coordinating vendor activities and monitoring contractual deliverables, reporting concerns to the COO.
• Identify project risks and barriers and bring them to the COO’s attention with relevant background information.
• Provide frequent, detailed communication on project status and milestones; prepare status reports, dashboards, and project-related deliverables for COO review; maintain detailed project documentation, meeting minutes, and action items.
• Compile and organize data from multiple sources to support reports prepared by leadership for the board of directors.
• Schedule, organize, and support project meetings with internal staff, health centers, and vendors, including agendas, minutes, and follow-up on action items.
• Track grant milestones and deliverables and support grant reporting, ensuring documentation requirements are met.
• Stay current on best practices, regulatory compliance, and operational improvements.
• Undertake additional project tasks assigned.

Minimum Job Requirements:

• Bachelor’s degree in healthcare or related field.
• 2+ years of project coordination or project-based work experience; PMP certification a plus but not required.
• Proficient in using SharePoint, Microsoft Outlook, Excel, PowerPoint, and Word.
• Experience with writing formal documents and policies.
• Experience working with EZCAP, Smartsheet, and Tableau preferred.

Salary Range:

• $90,000 – $110,000 annually
