Work Environment

This role is set within a supportive and collaborative work environment, primarily onsite at patients' residences. As you engage directly with patients, you will also have the chance to collaborate with an interdisciplinary team of healthcare providers to ensure the best outcomes for every individual. The typical work schedule is Monday to Friday, from 9:00 AM to 5:00 PM, allowing for a balanced professional life.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical abilities and mobility challenges to create personalized therapy plans.
• Administer therapeutic exercises and engage patients in rehabilitation activities designed to improve strength, flexibility, and overall physical capacity.
• Monitor and document patient progress, modifying treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families on treatment protocols, expected recovery timelines, and at-home exercises to promote self-management.
• Maintain clear, accurate, and timely documentation of patient interactions and progress reports in compliance with healthcare regulations.
• Coordinate with other healthcare professionals, including occupational therapists and nursing teams, to ensure comprehensive patient care.
• Provide support and insights during team meetings to enhance our therapy techniques and improve patient services.

Patient Population / Client Focus

Your primary focus will involve working with a diverse array of patients recovering from surgeries, injuries, and chronic pain conditions. This includes elderly patients looking to maintain their independence, individuals rehabilitating after orthopedic surgeries, and those needing assistance due to neurological conditions. Your expertise in human movement and rehabilitation will play a crucial role in their road to recovery.

Qualifications

• Active New York State Physical Therapist License.
• Bachelor's degree in Physical Therapy or an equivalent qualification.
• A minimum of two years of hands-on experience in a rehabilitation or home care setting, showcasing your ability to apply therapeutic techniques effectively.
• Strong communication and interpersonal skills to foster relationships with patients and their families.
• Ability to work independently and manage your schedule while ensuring high-quality patient care.

Preferred Experience

• Clinical experience in a home care setting.
• Familiarity with electronic health record (EHR) systems for maintaining patient documentation.
• Additional certifications such as PT Certification in a specialty area (i.e., geriatrics, pediatrics, orthopedics) would be advantageous.

Compensation and Benefits

The compensation package for this position ranges from $100,000 to $120,000 annually, commensurate with experience.
