Licensed Physical Therapist - Rehabilitation Services in Skilled Nursing Facility

Position Overview

We are seeking a dedicated and skilled Physical Therapist to join our rehabilitation team at Work It PT in Port Jervis, New York. In this full-time role, you will play a critical part in enhancing the recovery and mobility of residents in our skilled nursing facility. You will be responsible for developing and implementing tailored treatment plans that address each patient's unique needs, ultimately improving their quality of life and functional independence.

Work Environment

In your role as a Physical Therapist, you will work onsite in a welcoming and dynamic skilled nursing facility. The environment is designed to promote recovery, featuring state-of-the-art rehabilitation equipment and a variety of therapeutic spaces suited for individual and group therapy sessions. You will collaborate with a multidisciplinary team including occupational therapists, speech therapists, nursing staff, and physicians to ensure comprehensive care for our residents.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients' physical conditions and functional limitations, utilizing both objective and subjective data.
• Develop and implement individualized treatment plans based on comprehensive assessments, ensuring that they align with each resident's goals for recovery.
• Administer a variety of therapeutic modalities and exercises, including manual therapy techniques and pain management strategies, to facilitate healing and improve physical impairments.
• Monitor and document patient progress meticulously, adjusting treatment plans based on ongoing evaluations and changes in patient condition.
• Collaborate actively with interdisciplinary teams to optimize patient outcomes, ensuring an integrated approach to patient care.
• Educate patients and their families on home exercise programs and lifestyle modifications that support continued progress outside the facility.
• Adhere to all facility protocols, state regulations, and industry best practices while maintaining a safe and effective therapeutic environment.

Patient Population / Client Focus

Our patient population predominantly includes individuals recovering from surgery, traumatic injuries, strokes, and chronic conditions that affect mobility and daily functioning. You will have the opportunity to work with diverse age groups, tailoring your approach to meet the specific rehabilitation needs and rehabilitation goals of each resident, whether they require post-operative recovery or long-term care support.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited program, or a Doctor of Physical Therapy (DPT) degree.
• Active and unrestricted Physical Therapy license in the state of New York.
• Strong clinical assessment, communication, and interpersonal skills, with an aptitude for building rapport with patients and their families.
• A compassionate and patient-centered approach to care, demonstrating empathy and understanding in all patient interactions.

Preferred Experience

• At least two years of professional experience in a skilled nursing facility, rehabilitation center, or a similar healthcare setting.
• Experience utilizing evidence-based practices and rehabilitation methodologies tailored to various functional impairments.
• Familiarity with electronic medical records (EMR) systems to assist in effective documentation and patient management.
