This position is primarily onsite, providing therapy directly in patients' homes. Key Responsibilities

• Conduct comprehensive initial evaluations of patients' physical condition and mobility levels upon intake.
• Design, implement, and modify individualized treatment plans based on evaluations and patient goals.
• Administer a variety of treatments, including manual therapy techniques, therapeutic exercises, and the use of assistive devices to enhance patient mobility.
• Monitor patient progress and adjust treatment strategies as necessary, ensuring effective rehabilitation and optimal results.
• Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations.
• Work collaboratively with a multidisciplinary team, including physicians, nurses, and occupational therapists, to improve patient care and overall outcomes.
• Educate patients and their families on exercises, safety practices, and adaptive techniques to support continued progress at home.

Patient Population / Client Focus

In this role, you will focus on a diverse patient population, including post-operative recovery clients, individuals with chronic pain, and those managing various disabilities. Qualifications

• Applicants must possess an active New York State Physical Therapist license.
• A bachelor's degree in Physical Therapy is required. A Master's degree in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) is preferred.
• Strong verbal and written communication skills are essential for effective patient interaction and documentation.

Preferred Experience

• Minimum of 2 years of clinical experience in a physical therapy setting, with a preference for home healthcare or rehabilitation facilities.

Compensation and Benefits

The position offers a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience.
