Key Responsibilities

• Perform comprehensive patient assessments including wellness evaluations and follow-up visits for both chronic and acute health conditions.
• Utilize evidence-based practices to diagnose and treat a variety of illnesses effectively.
• Coordinate care efficiently with physicians, specialists, behavioral health providers, and social services to ensure comprehensive patient management.
• Deliver patient education on disease prevention, health maintenance, and the effective management of chronic conditions.
• Conduct routine medical procedures as permitted by your training and clinical expertise.
• Integrate data from our modern EMR system to track patient outcomes and ensure continuity of care.

Patient Population / Client Focus

As a Family Nurse Practitioner with Metro Line Medical Group, you will have the unique opportunity to serve a diverse patient population within the North Port community. Your focus will be on holistic health, addressing a spectrum of needs from pediatric to geriatric care, with a particular emphasis on chronic disease management and preventive healthcare practices.

Qualifications

• Current licensure in Florida as a Family Nurse Practitioner (or eligible).
• National certification by recognized bodies such as AANP or ANCC.
• A strong commitment to patient education, health equity, and the promotion of preventive care.
• A proven ability to thrive in a collaborative, team-based care environment while maintaining independence in clinical decision-making.

Preferred Experience

• A minimum of two years experience in outpatient family medicine is preferred, but newly graduated professionals are also encouraged to apply.
