RN Clinical Heart Transplant Coordinator

This job oversees the pre‑transplant, post‑transplant, and/or inpatient transplant clinical practice. It acts as a patient advocate and patient educator while promoting optimum patient independence, self‑care, and rehabilitation. The role includes independently monitoring certain patients, managing outpatient clinics, and periodically reviewing all patients with the physicians. Care and service are provided to individuals of all age groups. Education Graduate of an accredited school of nursing. Work Experience Preferred: Bachelor’s degree in nursing. Required: 3 years of related experience with an associate degree. Required: 2 years of related experience with a bachelor’s degree. Certifications Current registered nurse (RN) license in the state of practice. Basic Life Support (BLS) through the American Heart Association. Certified Clinical Transplant Coordinator (CCTC) through the American Board of Transplant Certification (ABTC) – preferred. Knowledge, Skills, and Abilities (KSAs) Knowledge of pre‑transplant, post‑transplant, and/or inpatient transplant clinical practice. Proficiency in using computers, software, and web‑based applications. Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process. Strong interpersonal skills. Job Duties Performs pre/post transplant clinical activities. Educates patients on procedures. Educates nursing staff/colleagues. Participates in activities for professional growth. Ensures compliance and participates in quality initiatives. Adapts behavior to the specific patient population, including but not limited to respecting privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style. Physical and Environmental Demands Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly. This requires walking or standing to a significant degree, or pushing and/or pulling arms or legs, or maintaining a production rate pace. Exposure to blood, body fluids, or tissue may occur, and the incumbent works with patients who have known or suspected communicable diseases, including the use of isolation rooms.
