Hybrid Clinical Lead: Care Manager for Complex Needs

Vynca is seeking a Clinical Lead Care Manager to coordinate care for clients with complex needs across providers. This hybrid role travels up to 5 days/week within the Sacramento County area, combining in-person visits with remote work to ensure timely and cost-effective access to care.
