Responsibilities Perform and document complex social services work, acting as liaison among the individual, legally authorized representative, and other stakeholders to ensure ongoing care, treatment, and support using person‑centered practices. Gather information to assess client support systems and service needs, including participation in interdisciplinary team meetings. Complete initial social history and maintain updated documentation. Support assessment of client rights, capacity to make decisions, and provide individual and group training to promote exercise of rights. Assist with coordination of admissions, transfers, transitions, and discharges. Work under moderate supervision with limited latitude for independent judgment. Perform other duties as assigned, including mandatory participation in disaster response or COOP activation as required. Oversee liaison functions, maintain open communication, secure consents, and access external resources. Provide assistance on guardianship issues, living options, end‑of‑life planning, and rights per state and federal laws. Oversee rights and capacity assessments performed by staff and review documentation. Assign staff to perform admissions, transfers, transitions, discharges, and furloughs to ensure access to services. Coordinate design and conduct of individual and group training on rights, voting, citizenship, and self‑advocacy skills. Participate in residence and family meetings, conduct formal presentations of facility processes. Essential Job Functions Attend work regularly and may be asked to work rotating, extended, or overtime shifts as needed. May serve on on‑call status on a rotating schedule to meet operational needs. Conduct interviews with individuals and stakeholders to determine needs and gather background information for assessments. Maintain communication and information exchange among the individual, authorized representatives, and agencies. Assist with guardianship, living option discussions, and end‑of‑life planning. Oversee completion of rights and capacity assessments and review documentation. Knowledge, Skills and Abilities Knowledge of individuals with intellectual and developmental disabilities and related growth, behavior, mental health, and developmental components. Knowledge of social work standards for case management, community resources, and practice principles. Skill in accountability for assigned property with limited financial responsibilities. Strong interviewing, assessment, planning, intervention, evaluation, case management, mediation, counseling, referral, problem‑solving, supervision, consultation, and advocacy skills. Ability to perform duties in accordance with the Texas State Board of Social Work Examiners Code of Conduct and the National Association of Social Workers Code of Ethics. Ability to communicate effectively in writing and verbally in individual and group settings. Licensure and Credentials Must maintain licensure with the Texas Board of Social Work Examiners. Required educational credential: Baccalaureate, Master, or Clinical Social Worker. At least one year of experience working directly with persons with mental health issues or completion of the Colorado State Hospital internship program. Initial Screening Criteria Baccalaureate Social Worker, Master Social Worker, or Clinical Social Worker. One year of experience working directly with persons with mental health issues, or completion of a state hospital internship program. Additional Information Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks (CANRS, Nurse Aid, Medication Aide, Employee Misconduct, LEIE). All applicants must be at least 18 years of age; Waco and Terrell Center for Youth applicants must be 21 years of age. Flexibility in work hours may be required; overtime and extended hours may be necessary. Compliance with HHSC immunization policy and state hospital procedures related to immunizations is required. Employees may be required to wear a mask and take protective measures if influenza vaccination not obtained. Military and veteran applicants are encouraged to apply if qualifying.
