Schedule:
- Monday: 3:00 PM – 12:00 AM
- Thursday: 2:00 PM – 12:00 AM
- Friday: 2:00 PM – 12:00 AM
- Saturday: 2:00 PM – 12:00 AM
- 35 Hours Per Week
- Weekends and holidays included
We are seeking an experienced Licensed Master Social Worker (LMSW) to provide psychosocial support and social work services in a healthcare setting. The LMSW will assess patient needs, provide supportive therapy, assist with discharge planning, and coordinate appropriate referrals and resources.
Key Responsibilities- Conduct comprehensive psychosocial assessments of patients.
- Provide supportive therapy and counseling based on patient needs.
- Assist patients and families with social and emotional concerns.
- Support discharge planning and coordinate appropriate referrals.
- Connect patients with appropriate community and healthcare resources.
- Maintain accurate and timely documentation of social work services.
- Collaborate with interdisciplinary healthcare teams to support patient care.
- Maintain confidentiality and follow applicable policies and professional standards.
- Master’s Degree in Social Work (MSW) required.
- Valid Licensed Master of Social Work (LMSW) license issued by the New York State Education Department (NYSED) required.
- Primary Source Verification of licensure required.
- Minimum 1 year of LMSW experience required.
- Experience conducting psychosocial assessments and supporting discharge planning preferred.
