Salary Range: - LMSW 70K - 80K

LCSW 90K - 100K

About the Role

We are seeking dedicated and compassionate Licensed Master Social Workers (LMSW) and Licensed Clinical Social Workers (LCSW) to join our growing Behavioral Health team. This hybrid role offers the opportunity to work within an integrated healthcare setting, providing high-quality mental health services while collaborating with multidisciplinary care teams.

Responsibilities

• Conduct comprehensive biopsychosocial assessments using evidence-based screening tools.
• Assess and evaluate patients referred by Primary Care Providers (PCPs) and other healthcare professionals.
• Diagnose and treat behavioral health conditions using DSM-5 criteria.
• Provide evidence-based therapeutic interventions, including:
• • Cognitive Behavioral Therapy (CBT)
  • Motivational Interviewing
  • Crisis Intervention
  • Interpersonal Psychotherapy
  • Behavioral Activation
• Develop, implement, and monitor individualized treatment plans.
• Conduct crisis assessments and risk evaluations while providing appropriate interventions.
• Collaborate with physicians, nurse practitioners, care managers, and behavioral health professionals to coordinate patient care.
• Participate in collaborative care meetings, case conferences, and clinical supervision.
• Facilitate referrals to community resources and specialty care programs.
• Maintain accurate and timely documentation in Electronic Health Records (EHR).
• Engage patients through outreach initiatives to improve access to behavioral health services.

Qualifications

LMSW

• Master's Degree in Social Work (MSW) from an accredited institution.
• Active New York State LMSW License.
• Experience in behavioral health assessments, treatment planning, and crisis intervention.
• Knowledge of DSM-5 diagnostic criteria.
• Proficiency with Electronic Health Records (EHR).
• Bilingual English/Spanish strongly preferred.

LCSW

Master's Degree in Social Work (MSW).

• Active New York State LCSW License.
• Minimum 1–2 years of clinical experience in behavioral or mental health.
• Experience providing individual and/or group therapy.
• Strong assessment, diagnosis, treatment planning, and crisis management skills.
• Experience working with Medicaid/Medicare populations is preferred.
• Bilingual English/Spanish preferred.
