Licensed Master Social Worker (LMSW) - Hybrid | Behavioral Health

Job Description Summary Hybrid behavioral health role in Bronx with one on-site day per week. The LMSW will provide patient assessments, behavioral health support, therapeutic interventions, crisis intervention, care coordination, and treatment planning in collaboration with primary care and multidisciplinary healthcare teams.
