Work Environment\n

This role is a remote position, allowing for flexibility while maintaining consistent communication with clients and the team. Although primarily remote, candidates should be available to visit clients as needed to conduct assessments and provide direct support.

Work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs. Flexibility in scheduling is crucial to accommodate visits and ensure uninterrupted care. On-call availability may also be necessary to address urgent situations or provide support outside of regular working hours.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, integrating emotional, mental, and social factors to tailor individualized care.

• Collaborate with seniors and their families to formulate personalized care plans that reflect their unique goals and circumstances.

• Maintain meticulous documentation of client assessments, care plans, progress notes, and all relevant correspondence.

• Stay informed about the latest in social work policies and practices, ensuring compliance with ethical standards.

Patient Population / Client Focus\n

The primary focus of this role is to support the senior population. This demographic often faces various challenges, such as isolation, health issues, and emotional distress. The LCSW will work closely with clients to understand their unique situations and develop actionable strategies that promote emotional and social well-being.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• At least two years of experience in a social work role, ideally in a geriatric or community health setting.

Compensation and Benefits\n
