About the role Position: Family Well-Being Coordinator Reports to: Director, Housing Justice Department: Housing Justice Pay Rate: $29-$37 hr. Position Type: Full Time, Hourly The Family Well-Being Coordinator (FWC) serves as the primary care coordinator and family support professional for assigned children and families participating in Community Action Marin's Whole Family Approach. The position integrates family engagement, case management, and CalAIM Enhanced Care Management (ECM) to improve health, educational, and social outcomes for children and their caregivers. Working collaboratively across Children & Family Services, Housing Justice, Health & Nutrition, Economic Justice, Behavioral Health, and community partners, the Family Well-Being Coordinator provides comprehensive care coordination, resource navigation, and family-centered planning that addresses social drivers of health and promotes long-term family stability. Operating within California Department of Education requirements, Head Start Performance Standards, Community Care Licensing regulations, California Department of Health Care Services (DHCS) CalAIM guidelines, and Community Action Marin policies, the Family Well-Being Coordinator ensures coordinated, culturally responsive, trauma-informed services that strengthen families and improve child outcomes. Essential Duties Family Engagement & Whole Family Services Establish and maintain trusting, culturally responsive, strengths-based relationships with assigned children, families, and caregivers. Serve as the primary point of contact for families by providing ongoing support, advocacy, education, and resource navigation. Complete comprehensive family assessments to identify strengths, barriers, goals, and needs utilizing a trauma-informed and family-centered approach. Develop collaborative Family Partnership Agreements and individualized service plans that support family self-sufficiency and child well-being. Support enrollment, orientation, transitions, attendance, and ongoing participation in Community Action Marin programs. Empower families to achieve personal and family goals through coaching, education, and coordinated support services. Promote family engagement opportunities that strengthen parent leadership, child development, and school readiness. CalAIM Enhanced Care Management (ECM) Serve as the assigned Care Coordinator for eligible CalAIM participants. Conduct comprehensive ECM assessments in accordance with DHCS guidelines. Develop, implement, monitor, and revise individualized Care Plans based on identified medical, behavioral health, developmental, and social needs. Coordinate services among healthcare providers, managed care plans, behavioral health providers, housing agencies, schools, regional centers, and community organizations. Assist families in accessing Medi-Cal benefits, CalAIM services, specialty care, behavioral health services, developmental evaluations, and community-based supports. Monitor family progress toward care plan goals and adjust interventions as family needs evolve. Ensure all ECM activities comply with DHCS documentation, timeliness, billing, and quality requirements. Care Coordination & Community Partnerships Coordinate multidisciplinary services that address housing stability, food security, transportation, employment, health care, early childhood education, and other social drivers of health. Participate in multidisciplinary case conferences, family team meetings, and cross-program planning sessions. Build collaborative partnerships with healthcare providers, school districts, managed care organizations, county agencies, and community partners. Serve as an advocate for families while promoting equitable access to services and reducing systemic barriers. Facilitate referrals across Community Action Marin programs to ensure integrated whole-family service delivery. Program Compliance & Documentation Maintain complete, accurate, and timely documentation within ChildPlus, Brightwheel, EmpowOR, SharePoint, and other agency systems. Complete assessments, case notes, care plans, referrals, follow-up documentation, and required reports within established timelines. Monitor family files to ensure compliance with Head Start Performance Standards, California Department of Education requirements, Community Care Licensing regulations, DHCS CalAIM standards, and Community Action Marin policies. Maintain confidentiality in accordance with HIPAA, FERPA, applicable privacy laws, and agency policies. Monitor program outcomes and utilize data to improve service delivery and family success. Collaboration & Organizational Support Collaborate with Children & Family Services, Housing Justice, Economic Justice, Health & Nutrition, Behavioral Health, and other Community Action Marin departments. Promote Community Action Marin's Whole Family Approach by fostering collaboration across programs. Participate in staff meetings, professional development, coaching sessions, and required trainings. Assist with continuous quality improvement initiatives and implementation of best practices. Promote Community Action Marin's Whole Family Approach, as a Success Coach, by fostering collaboration across programs. Support outreach, recruitment, family engagement events, and community activities Qualifications Education: A high school diploma or equivalent (GED) is required in accordance with Community Action Marin employment standards. Associate's degree in Social Work, Human Services, Child Development, Public Health, Psychology, Family Studies, Public Administration, or a closely related field required. Bachelor's degree in Social Work, Public Health, Human Services, Child Development, Healthcare Administration, Psychology, or a closely related field preferred. Equivalent combinations of education, lived experience, and progressively responsible experience may be considered. Experience: Minimum three (3) years of progressively responsible experience providing family support, care coordination, case management, social services, healthcare navigation, early childhood services, or community-based programs. Experience working with culturally diverse, low-income, and underserved children and families. Demonstrated experience coordinating services across multiple agencies, healthcare providers, and community partners. Experience maintaining confidential case documentation and managing complex caseloads. Experience working in Head Start, Early Head Start, CSPP, CalAIM Enhanced Care Management (ECM), Medi-Cal Managed Care, behavioral health, housing services, or other publicly funded human services programs preferred. Skills and Abilities: Comprehensive knowledge of family-centered practices, case management, care coordination, and community resource navigation. Working knowledge of CalAIM Enhanced Care Management (ECM), social drivers of health, and whole-family service delivery models preferred. Knowledge of Head Start Performance Standards, California Department of Education requirements, Community Care Licensing regulations, and applicable federal and state regulations governing family services. Demonstrated ability to build trusting, culturally responsive, and strengths-based relationships with children, families, community partners, and interdisciplinary teams. Strong organizational, time management, and case management skills with the ability to manage multiple priorities and deadlines. Excellent written, verbal, and interpersonal communication skills. Strong analytical, problem-solving, and critical thinking abilities. Ability to exercise sound independent judgment and maintain professional boundaries when addressing complex family situations. Ability to work collaboratively across multiple departments and community organizations. Bilingual English/Spanish required. Valid California Driver's License, reliable transportation, and proof of current automobile insurance required. Software: Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint). Experience utilizing electronic case management systems, including ChildPlus, Brightwheel, EmpowOR, SharePoint, or similar client information systems preferred. Ability to learn and effectively utilize agency software, reporting platforms, and electronic documentation systems.
