Social Worker

Job Description The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care with the appropriate care team, assess crisis situations to provide needed resources and referrals, and provide family and individual treatment as indicated. This position works collaboratively with patients, their significant others, families, healthcare providers, insurers, community resources, and all other involved parties. The role provides care appropriate to the established plan of care within the assigned patient population. Sign‑on Bonus External applicants with 1+ year of experience may receive a $2,500 sign‑on bonus. Job Specifics Pay Range Clinical: $35.60 – $54.93 (Exempt) Pay Range Certified: $32.34 – $49.94 (Exempt) Benefits Eligible: Yes FTE: Full time Shift: 36 hrs/week, Night shift Essential Functions Diagnose and treat various mental, emotional, and behavioral disorders, modifying care plans to reflect changes in patient status. Refer patients to other services (community services, treatment programs, support groups, psychiatrists, or other mental health providers) to improve their well‑being. Work with a multi‑disciplinary team to develop a discharge plan and ensure continuity of care. Organize and prioritize daily work by assessing new, current, and discharging patient needs. Perform behavioral health consultation, conduct psychosocial assessments, and provide supportive, crisis, bereavement, and other interventions as necessary. Educate, communicate, and coordinate with multiple patient care teams and outside agencies to ensure quality care, timely discharge, and resolution of delays. Provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events. Minimum Qualifications Master's Degree in Social Work or related field from an accredited institution. Current state licensure as CSW, ACMHC, LCSW, LMFT, or CMHC. Excellent written and verbal communication skills. Computer skills, including email and word processing. Preferred Qualifications Experience working with patients, families, and treatment teams. Physical Requirements Ongoing need to see and read information, labels, and assess patient needs; operate monitors. Verbal communication and hearing required, particularly during emergency situations. Manual dexterity for complex equipment, frequent computer use, and typing. Ability to lift and transfer patients, and to lift and carry supplies up to heavy weights. Ability to walk and assist with transporting patients and moving equipment, often in crowded environments. Must be able to stand at a stationary position for extended periods. Location Intermountain Health St George Regional Hospital, St George, Utah. Scheduled Weekly Hours 36 hours.
