JOB SUMMARY
The LVN utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio-economic background and response to medical regimen and treatment. Additionally, they are required to learn and apply the Patient-Centered Medical Home Standards and Ten Building Blocks of Primary Care. In collaboration with the Medical Home health care providers, nurses, and medical assistants the LVN will also assume clinical responsibility for administration of medications/vaccines, lab result review and monitoring, treatments and procedures pertinent to pediatric & adult primary care such as ear irrigations, wound care and urethral catheterization, as well as patient education.
This individual will utilize their good customer service and communication skills daily via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers. This employee will utilize computer skills in Apex and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. They are required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrating UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.
The Sr Licensed Vocational Nurse will be primarily located at Bayfront, however, must have flexibility to cover multiple UCSF medical center Primary Care Clinic sites.

Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath. C. Learns how to respond in a code blue scenario and understands use of emergency equipment. D. Reports emergency or unsafe patient care conditions on the Incident Reporting System and to the management team. | |

Works cooperatively and participates with the health care team to discuss patient’s alternatives to health care services and to develop short and long range goals to meet the patient’s needs. B. Provides individualized nursing care which responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient. B. Demonstrates ability to provide appropriate care based on the needs of a specific individual, including that individual’s age. Sets up and maintains cleanliness of exam rooms including periodic exam room disinfection and organization and removal of expired supplies. D. Performs intake process and engages in inreach and outreach to support practice quality initiatives and to support the model of population management. Provides for patient’s physical and emotional privacy. E. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments. F. Assists as needed with diagnostic and therapeutic procedures and treatments. This includes vaccines, injections, wound care and phlebotomy. G. Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC on CLIA waived tests and reports any tests out of QA range to supervisor. H. Uses appliances and equipment correctly and maintains safe practice. I. Evaluates and documents patients’ responses to nursing care in the electronic medical record, including pre/post procedures and treatments. Uses knowledge of cognitive, physical, emotional and chronological maturation process in evaluating care for all ages of patients. J. Administers and documents the administration of medication while maintaining current knowledge of pharmacology and competency in modes of administration. K. Documents telephone messages and chart notes in a thorough and grammatically correct manner. L. Responds appropriately in emergencies. Is knowledgeable about the contents of the code blue cart and/or the emergency kits and can initiate and assist in CPR. Understands the procedures for medical emergencies, code blue and other emergency/disaster situations. Participates in safety drills. M. Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice. Complies with activities mandated by TJC/Title 22/NCQA. N. Completes annual competency testing as specified by the individual practice | |
10% | Coordination/Continuity/Communication A. Organizes activities based on the needs of patients while maintaining the effective flow of patients through the practice. Adjusts clinical support activities to reflect changes in priorities during practice sessions. B. Maintains open communication and facilitates continuity of care for patients/and or family with members of the health care team through phone contact, verbal interaction. C. Maintains accurate, records and documentation which include patient complaints, nursing observations, interventions and care plan, nursing actions/teaching performed and patient response to the action. D. Obtains, records, and conveys information, clearly and concisely. E. Demonstrates courtesy, common sense, and follow-through in all telephone/electronic communications, maintaining the Telephone Standards as established by the Medical Center. F. Completes all tasks within assigned InBasket to include cross coverage of InBaskets of other teams and other practices across primary care. G. Fosters/maintains positive public relations (patients, family, visitors and staff). Uses effective communication skills with patients and staff; demonstrates proper telephone techniques and etiquette; shows sensitivity to differences of culture; demonstrates a positive and supportive manner in which patients perceive interactions as positive and supportive. H. Demonstrates ability to work as a team member with providers, supervisors, fellow nursing staff and clerical/administrative staff within the team structure, with the practice team and in cross organization relationships. I. Participates in daily huddles to support efficient and patient centered care. J. Demonstrates Everyday PRIDE values and the AIDET fundamentals of communication in all patient care stakeholder interactions. | |

Qualifications
REQUIRED QUALIFICATIONS
- Minimum of 3 years LVN experience
- Experience in an outpatient primary care setting
PREFERRED QUALIFICATION
- Epic experience
- Assisting in minor procedures in a clinic setting
REQUIRED LICENSES/CERTIFICATIONS
- California Licensed Vocational Nurse (LVN)
- Current American Heart Association CPR Certification
- Must have blood withdraw/ venipuncture certification
