Social Worker

Schedule: Monday–Friday | 8:30 AM – 4:30 PM Overview We are seeking a compassionate Hospice Social Worker to provide emotional support, counseling, and resource coordination for patients and families receiving hospice care. This role works closely with an interdisciplinary team to ensure patients and their loved ones receive comprehensive psychosocial support throughout their care journey. Key Responsibilities Conduct psychosocial assessments and develop individualized care plans. Provide counseling and emotional support to patients and families. Connect patients and families with community resources and support services. Participate in interdisciplinary team meetings and care coordination. Maintain accurate and timely documentation. Required Qualifications Master's Degree in Social Work (MSW) from an accredited program. Current New Jersey Social Work License. Minimum 1 year of post-master's social work or counseling experience. #J-18808-Ljbffr
