Work Environment

In this onsite role, you will deliver physical therapy services directly in patients' homes, allowing for a personal connection that is often missing in traditional clinical settings. As part of a dynamic health care team, you'll collaborate closely with other healthcare professionals and caregivers to create holistic treatment plans tailored to each client's needs. The work schedule is Monday to Friday, ensuring a balanced professional and personal life.

Key Responsibilities

• Direct physical therapy treatment, focusing on preventing disability, alleviating pain, and restoring and improving muscle function, following the individualized Plan of Care.
• Oversee the implementation of treatment plans by Physical Therapy Assistants, ensuring that services are delivered appropriately and effectively.
• Conduct thorough evaluations of patients' functional abilities through diagnostic testing and assessments of muscle, nerve, joint, and overall functional capabilities.
• Assess the home environment, providing recommendations aimed at increasing patient independence during rehabilitation.
• Deliver prescribed physical therapy services, including manual therapeutic exercises, while instructing and motivating patients in non-manual exercises and functional activities.
• Administer treatments involving traction, massage, and various physical agents according to patient care requirements.
• Evaluate, fit, and adjust prosthetic and orthotic devices, making necessary recommendations to the referring physician.
• Contribute to care plan development, review, and updates, collaborating closely with an interdisciplinary team to ensure comprehensive treatment delivery.
• Document medical records including initial evaluations, progress notes, and discharge summaries, submitting them promptly for inclusion in the client's clinical records.
• Participate in case conferences and interdisciplinary team meetings to evaluate and plan comprehensive patient care.
• Instruct clients and their families on the use and care of physical therapy devices as well as the intricacies of their ongoing therapy program.
• Maintain accessibility to Physical Therapy Assistants during treatment sessions, ensuring ongoing supervision and support.
• Complete other responsibilities as assigned, contributing to our organizational objectives.

Patient Population / Client Focus

Your role as a Physical Therapist focuses on a diverse patient population requiring home care services, which may include elderly individuals, those recovering from surgeries, and patients with chronic conditions or disabilities. You will be central to helping these clients regain mobility, improve functionality, and enhance their overall well-being in the comfort of their homes.

Qualifications

• A graduate of an accredited physical therapy program.
• A current and valid state PT license in good standing.
• Minimum of one year of physical therapy experience, preferably in acute care or home care settings.
• Current CPR certification and a valid driver's license with automobile insurance.
• A creative thinker with strong decision-making skills, capable of leading change for better patient outcomes.
• A passion for clinical excellence and a commitment to quality care.

Preferred Experience

• Experience in coordinating care with a multidisciplinary team and actively participating in interdisciplinary team meetings.
• Previous experience in home health settings providing therapy services.
• Expertise in the use of technology for documentation and patient tracking.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience. We provide a comprehensive benefits package including health insurance, paid time off, and support for continued education and professional development.
