Urgent Care Nurse Practitioner

Adobe Population Health is seeking a Nurse Practitioner to join the Adobe Provider Group, delivering telemedicine-based primary care across Arizona. You will assess patients, plan care, and coordinate services for improved outcomes, with occasional in-home visits within Arizona. Monday–Friday, 8:00 a.m.
– 5:00 p.m., remote work possible; travel may be required for in‑home support. The role emphasizes quality care, HIPAA compliance, and collaboration with care teams to meet contract requirements
