Location: Mesa / Apache Junction / Gilbert, AZ Compensation: $19-$21/hr Status: Full-Time, W2 Employee Schedule: Monday - Friday, 8 AM - 5 PM About The Role We are seeking a compassionate and dynamic Bridge Care Coordinator to join our Bridge Program Team. This position is a critical part of enhancing the care experience by bridging the gap between home health and hospice services. You'll help provide peace of mind to families and create meaningful connections with the seniors you serve. This unique program ensures a smoother transition for our patients by providing consistent, compassionate support. The Bridge Program redefines post-acute care with an added layer of comfort and continuity. Your Key Responsibilities Conduct scheduled in-home non-clinical visits with Bridge Patients. Educate families about Aegis Healthcare's continuum of services. Connect patients to valuable external resources. Bridge the gap between home health and hospice by offering guidance and support to patients and families. Offer companionship that fosters trust and well-being for seniors. Qualifications Be at least 18 years old. At least 1 year in healthcare marketing (preferred). Possess a valid driver's license, clean driving record, and reliable automobile.
