Key Responsibilities\n

• Maintain a strong line of communication with healthcare providers, accurately reporting patient progress, concerns, and needs to ensure continuous care.

• Collaborate with clients’ families or support systems to create effective treatment and discharge plans that involve the active participation of all parties.

• Document comprehensive and timely clinical notes and progress reports while adhering to departmental policies to maintain the integrity of patient records.

• Engage in multidisciplinary meetings to contribute to treatment planning and coordinate efforts towards patient-centered care and successful discharge outcomes.

• Assist clients with care coordination during their discharge process, working within various local systems to ensure they have access to continued necessary services post-treatment.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals facing various mental health challenges, including anxiety, depression, and substance abuse. Our focus on a recovery-oriented model emphasizes empathy, empowerment, and resilience, providing a safe space for clients to express their thoughts and feelings. By actively listening and utilizing your clinical expertise, you will help clients develop coping strategies and skills essential for their wellness journey.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Clinical Psychology.

• Active and unrestricted licensure as an LCSW, LMFT, LPC, or comparable credential.

• A minimum of three years of experience in an outpatient setting is essential to understand the complexities of client needs and treatment planning.

• Proficient computer skills, including familiarity with electronic medical records, word processing, spreadsheets, and various communication tools.

• A self-directed, organized individual capable of working independently while being a part of a larger team.

Preferred Experience\n

Experience in utilizing evidence-based practices and therapeutic modalities is highly desirable. Familiarity with trauma-informed care and crisis intervention techniques will enhance your effectiveness in this role. Previous experience in a community health setting is a plus, as it prepares you for the diverse challenges that may arise while working with our client population.
